We Have Televisits

For established patients in Illinois, we can use our medical software to connect by video conference, though appointments in the office provide more evaluation options. 

 

As of 2023, FIRST-TIME NEW patient or OUT-OF-STATE patient televisits may NOT be covered by insurance.  If you are interested in making such a televisit, click here to download a televisit charge summary and authorization for billing.  Please complete and return to us prior to your appointment.
